Exhibition Game Request Travel Permits   HEO Travel Permit - Application Form Edit the saved file and submit to: travel@cumberlandminorhockey.ca along with a copy of the team’s official roster as approved by the registrar.  The details in the travel permit must match the details on the Roster EXACTLY. Do not add individuals or information on the permit if they do not also appear on the roster.   List the players in the exact same order as on it appears on the roster. This will help facilitate the approval process.
